The Individualized Family Service Plan (IFSP) is a written plan created to meet the individual needs, concerns, and priorities of individual children, from birth to age 3, and their families. The IFSP must include information in the following categories: Levels of development in cognitive, physical (including vision, hearing and health), communication, social– emotional, and adaptive areas.
